Our Adult Mental Health Targeted Case Management (TCM) service offers individuals a unique, personalized approach to supporting their mental health journeys. TCM focuses on building strong community connections and helping clients navigate complex systems such as housing, transportation, healthcare, finances, and self-care resources. Case managers work closely with individuals to coordinate services, reduce barriers, and promote independence and stability. Through advocacy, guidance, and ongoing support, we help clients access the tools and services they need to thrive in their daily lives.

Targeted Case Management (TCM)

Our Adult Mental Health Targeted Case Management (TCM) service offers individuals a unique, personalized approach to supporting their mental health journeys. TCM focuses on building strong community connections and helping clients navigate complex systems such as housing, transportation, healthcare, finances, and self-care resources. Case managers work closely with individuals to coordinate services, reduce barriers, and promote independence and stability. Through advocacy, guidance, and ongoing support, we help clients access the tools and services they need to thrive in their daily lives.

When do you meet with TCM?
TCM clients typically meet with their provider 1-4 times a month and can coordinate services by phone or telehealth.

Targeted Case Management services that can be provided:

Assist in maintaining benefits such as cash assistance, food stamps, applying for emergency assistance, and accessing community vouchers
Assist in finding new housing as well as maintaining current housing as needed
Linking client to other agencies for additional services or financial assistance
Involves extensive coordination with the clients care team to establish new appointments, maintaining current appointments, and setting up medical cab rides
Accessing services for transportation
Referring to appropriate agencies, and assisting in navigating the court system through paperwork and appropriate phone calls
Assist in locating new employment
Care coordination with other providers

What Targeted Case Management is not:

Cleaning service including completing chores, laundering clothes, moving assistance
Transportation to shop or run errands
Housing placements service or assist in obtaining
housing vouchers
Employment referral services

MNSure Navigation

WebMed’s MNsure Navigation services help individuals and families understand their health insurance options and enroll in coverage that best fits their needs. Our certified navigators provide personalized assistance to simplify the enrollment process and reduce barriers to accessing care. We offer support with:

  • Exploring and comparing MNsure health plan options

  • Completing applications and renewals accurately and on time

  • Understanding eligibility, premiums, and financial assistance programs

Our goal is to make health coverage accessible, clear, and stress-free so clients can focus on their health and well-being.

Sliding Fee Schedule

WebMed offers a Sliding Fee Schedule to improve access to care by making mental health and recovery services more affordable for individuals and families in our community. Our goal is to ensure that clients are not turned away due to financial hardship while continuing to provide high-quality, compassionate care. All available insurance and funding resources must be fully explored before a client is considered for the Sliding Fee Schedule. Eligibility is determined through a fair review process based on verified financial information.

  • Clients are not denied services based solely on inability to pay

  • All insurance and funding options must be exhausted prior to approval

  • Income and household verification are required for consideration

  • Fees are adjusted based on documented financial need
