WebMed is seeking a Family Nurse Practitioner (FNP) to join our outpatient mental health and substance use clinic. This position will work a split schedule between our Duluth and Cloquet locations and assist in overseeing our Treatment Resistant Depression Programming. Tasks will include providing consultation, education, treatment and follow up for TMS and esktamine services. Experience working with TMS and/or Spravato is not required and on-sight training can be provided to the right candidate.

Essential Duties and Responsibilities including but not limited to:

General

Provides mental health related services to a diverse client population at the clinic system’s main office
The provision of clinical services including conducting treatment resistant depression programming evaluations, treatment planning, medication management, medical cannabis programming, suboxone management, and discharge planning for patients of the clinic
Providing intermittent didactics within clinic’s training program if desired
Compliance with clinic documentation standards
Compliance with licensing board rules, applicable codes of ethics, and DHS regulations
Maintain professionalism in all interactions with staff, patients, and business associates of clinic
Carrying assigned treatment caseload
Ability to provide medical services under independent licensure
Compliance with clinic and training program policies and procedures, if applicable
Follow facility policy and protocol
Coordination with a multidisciplinary team.
Provide a safe, friendly, approachable environment for clients to enter and build rapport.
Be an active member of the WebMed team to promote a comfortable climate, culture and environment while seeking the best outcomes for the clients we serve.
Coordination with providers/team members from other facilities for client care.
Act as a preceptor for potential students that are seeking clinical experience.
Other duties as assigned

Treatment Resistant Depression (TRD) Program Champion

Develop a deep understanding of the TRD programming offered at WebMed: esketamine & TMS
Able to answer programming questions to staff, clients and the public regarding programming
Work with esketamine (Spravato) representatives for prior authorization and client billing questions
Understand and able to decipher the different prior authorizations needed from different insurances for clients seeking esketamine or TMS treatment.
Provide consultation for clients seeking Treatment Resistant Depression Programming at WebMed under the supervision of a licensed provider
Assist with ordering esketamine for buy & bill clients
Coordinating with pharmacies to ensure esketamine is delivered on time
Creating, updating and maintaining Prior Authorizations for esketamine & TMS within the clients EHR.
Reviewing insurance to ensure the WebMed has the proper Coordination of Benefits prior to starting the Prior Authorization process.
At the beginning of each month for each client that is currently active in the TRD programming, a review of the client’s insurance is to be completed to ensure accurate insurance on file for the client
Able to verbalize both program schedules, basic rule in/out needs, insurance coverage and enrollment to the public at different events.
Champion Treatment Resistant Depression Programming and act as a resource for clinicians, staff, clients, client family, outside providers and community
Other duties as assigned

Transcranial Magnetic Stimulation (TMS)

Provide supervision, documentation and /or procedure for transcranial magnetic stimulation (TMS) after completion of TMS training.
Discuss and explain the programming, criteria, expected prognosis, process and evaluation
Rule in/out TMS evaluation
Refer to physician for further evaluation if needed
TMS initial mapping and redetermination
Following progress of TMS clients
Referring for further medication management with psychiatry

Esketamine (Spravato)

Discuss and explain programming, criteria, expected prognosis, process and evaluation
Refer clients to outpatient SUD if appropriate
Review insurance requirements
Spravato REMS registration
Ordering of Spravato for specific clients
Monitoring daily count of Spravato
Wasting unused/expired Spravato with REMS
Providing education for administration of Spravato
Assessing clients mental status and blood pressure prior to client self administration of Spravato
Assessing blood pressure 40minutes after Spravato administered
Assessing blood pressure 2 hours after Spravato administered and approving discharge from facility
Follow up appointment after first month of Spravato treatment and develop follow up plan with client

Suboxone

Providing follow up appointments face to face or telehealth for clients receiving suboxone.
Review with SUD team for clients participation in outpatient treatment
Coordinate with multidisciplinary team and any additional outside resources as appropriate for ongoing MAT management
Provide care for clients seeking suboxone induction, discussing plans with patient and team.

Follow Up Care

Provide follow up medication management appointments as directed by the Medical Director that fall within the scope of practice of the Family Nurse Practitioner

Qualifications

Graduate of a school or nursing and accredited Family Nurse Practitioner training program.
BC/BE through the AANP or ANCC
Licensed or ability to obtain APRN license with the Minnesota Board of Nursing
Ability to obtain and maintain DEA certification for the State of Minnesota
Obtain and maintain BLS/CPR certification
Meet threshold criteria/qualifications for Credentialing and Privileges (if required)
Ability to successfully complete the required background check process
Evidence of a high level of professionalism within the field
Strong clinical acumen and judgment
Ability to base clinical assessment and intervention methods on science and professional best practice
Ability to work both independently and as part of a multidisciplinary treatment team
Excellent interpersonal and written/verbal communication skills
Ability to respect diversity and integrate issues of culture into case conceptualizations
Ability to engage with patients who may be ambivalent about treatment
Ability to adapt interventions to patients’ strengths, skills, and developmental levels
Commitment to empowering patients to solve their own problems when necessary
Ability to intervene decisively, when necessary, to protect patient safety
Ability to set appropriate boundaries and limits for best practice
Ability to comply with clinic documentation standards

Preferred Qualifications

At least 2 years of APRN practice experience in psychiatric clinical practice is preferred.
Licensed and experienced in treating pediatric patients is also preferred, but not required.
Documented 2 years freedom from chemical use problems.
Exhibit strong commitment to the client’s needs and the mission statement of WebMed.
Excellent written and verbal skills.
